Instructions for Use

• All forms must be printed clearly, filled in capital letters or typed and signed by the authorized person (coach, referee or federation head).

• Forms must be submitted by the deadline specified in the competition or camp circular.

• Players or teams submitting incomplete or altered forms will be considered ineligible.

• Only AGF-approved formats will be accepted. Modified or edited forms will be rejected.

• Where applicable, scanned/digital versions of the signed forms may be emailed to the official AGF email ID for pre-verification.
